DESCRIPTION:FCMS (NW) Ltd has been commissioned by Lancashire County Cou
 ncil as a Community Outreach Provider to deliver NHS Health Check. The 
 offer is open to those who live or work in Lancashire. The NHS Health C
 heck programme is It is completely free and is open to anyone aged between
  40-74 years who doesn’t have an existing cardiovascular disease and has
 n’t had a check in the last 5 years. It takes 20-30 mins to complete and
  consists of the following:\n \n\n\n 	Blood pressure check\n 	Pulse check
 \n 	BMI/height/waist measurement\n 	Cholesterol test\n 	Blood sugars test\
 n 	General questions in relation to family &amp\; medical history\, lifest
 yle questions regarding exercise\, alcohol\, smoking\n\n \nThe check give
 s a QRISK score which is the likelihood of contracting a cardiovascular di
 sease within the next 10 years. We discuss the results with the employee a
 nd provide a booklet which documents their results and gives additional ad
 vice regarding lifestyle choices. Results are shared with their GP. We wil
 l ask the GP to follow up in the event of adverse results and can sign pos
 t people to lifestyle providers such as Smoke Free Lancashire\, Healthy W
