DESCRIPTION:FCMS (NW) Ltd has been commissioned by Lancashire County Cou
 ncil as a Community Outreach Provider to deliver NHS Health Check. The 
 offer is open to those who live or work in Lancashire. The NHS Health C
 heck programme is It is completely free and is open to anyone aged between
  40-74 years who doesn’t have an existing cardiovascular disease and has
 n’t had a check in the last 5 years. It takes 20-30 mins to complete and
  consists of the following:\n\n&nbsp\;\n\n 	Blood pressure check\n 	Pulse 
 check\n 	BMI/height/waist measurement\n 	Cholesterol test\n 	Blood sugars 
 test\n 	General questions in relation to family &amp\; medical history\, l
 ifestyle questions regarding exercise\, alcohol\, smoking\n\n&nbsp\;\n\nTh
 e check gives a QRISK score which is the likelihood of contracting a cardi
 ovascular disease within the next 10 years. We discuss the results with th
 e employee and provide a booklet which documents their results and gives a
 dditional advice regarding lifestyle choices. Results are shared with thei
 r GP. We will ask the GP to follow up in the event of adverse results and 
 can sign post people to lifestyle providers such as Smoke Free Lancashire\
 , Healthy Weight management services etc. if required.\n\nThe van will be
  parked outside One Beyond on St James Street.
